Prevalence of Gastric Ulcer in Patients with Acute Pancreatitis and Its Risk Factors

Abstract
Objectives: To evaluate the prevalence of gastritis and gastric ulcer in patients with actue pancreatitis on the basis of gastrofiberoscopy, and the risk factors of development of gastric ulcer as well.

Materials and Methods : Total of 115 patients who diagnosed as acute pancreatitis and underwent gastrofiberoscopy from Jan 2001 to Dec 2007 in Konyang university hospital were enrolled. The medical records of the patients were reviewed.

Results: Mean age of the patients (n = 115) was 54.9 years and the male to female ratio was about 2:1. The causes of acute pancreatitis were excessive alcohol ingestion (n = 56, 48.6%), idiopathic (n = 30, 26.0%), gallstones (n = 26, 22.6%), and hyperlipidemia (n = 3, 2.6%) in the order of frequency. The most common endoscopic finding was gastritis
(72, 62.6%). And gastric ulcer (16, 13.9%), reflux esophagitis (7, 6.0%), hemorrhagic gastritis (3, 2.6%) was also noted. Among the gastric ulcers, 2 cases were hemorrhagic. Among the risk factors of development of gastric ulcer, causes of acute pancreatitis and presence or absence of pancreatic necrosis were statistically significant. But, age, gender, Ranson score, severity score in CT weren¡¯t significant.

Conclusion: It seemed useful to evaluate gastrofiberoscopy in patients with acute pancreatitis in the acute stage, to differentiate the cause of pain in patients with ulcer-like pain, and to start acid suppressive therapy, especially in
patients with pancreatic necosis revealed on CT.
